The School of Veterinary Medicine at the University of California, Davis, is gathering information to learn more about the role of dogs in the lives of people with disabilities. The questionnaire is designed for people with disabilities (other than visual or hearing disabilities), their family members, and caregivers.
Non-dog owners can also take this survey. Your responses will help improve veterinary services for service dogs.
